OCR description
The covered entity (CE), Northwestern Memorial HealthCare, reported that its business associate (BA) was the victim of a cyber-attack affecting the protected health information (PHI) of 201,197 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications prescribed, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. The CE also established a call-center and provided credit monitoring and identity protection services for affected individuals. In response to the breach, the CE terminated its business relationship with its BA.
