OCR description
The business associate (BA), PracticeMax, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 685,574 individuals. The PHI involved included names, diagnoses, addresses, dates of birth, Social Security numbers, financial information, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional technical safeguards.
