OCR description
The covered entity (CE), CentraCare Health System, reported that its business associate (BA) was the subject of a ransomware attack that affected the protected health information (PHI) of 842 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, medications, and diagnoses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE and BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect PHI.
