Coastal Family Health Center, Inc

The covered entity (CE), Coastal Family Health Center, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 65,860 individuals. The PHI involved included names, addresses, dates of birth; Social Security numbers, diagnoses, lab results, medications, and other treatment and financial information. The CE notified HHS, affected individuals, the media, and posted substitute notice its website. In response to the breach, the CE offered free credit monitoring services and implemented additional technical safeguards, revised it policies and procedures, and retrained its workforce.
