OCR description
Hope Medical reported that boxes that contained the protected health information (PHI) of 500 individuals were improperly disposed of. The PHI involved included names, telephone numbers, addresses, dates of birth, Social Security and drivers’ license numbers, financial and claims information, lab results, medications prescribed, and other treatment information. During the investigation, Hope Medical changed its business status and was subsequently not a covered entity. The investigation was therefore, closed.
