Triangle Women's Center

The covered entity (CE), Triangle Women’s Center, reported that an employee inadvertently sent an email containing the electronic protected health information (ePHI) of 978 individuals to an unauthorized individual. The ePHI involved included names, addresses, telephone numbers, and billing information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards and retrained its workforce. OCR obtained assurances that the CE implemented the corrective actions noted.
