OCR description
The covered entity (CE), Lake County Health Department and Community Health Center, reported that the protected health information (PHI) of 705 individuals was shared over the Internet. The PHI involved included names, addresses, dates of birth, email addresses, telephone numbers, and diagnoses. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE strengthened its administrative safeguards to better protect PHI.
