OCR description
The covered entity (CE), Starling Physicians, reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 2,808 individuals. The PHI involved included clinical, demographic, and financial information. Starling Physicians notified HHS, affected individuals and the media of the breach. In response to the breach, the CE implemented additional administrative and technical safeguards.
