OCR description
The covered entity (CE), King County Public Hospital District No. 2, dba EvergreenHealth, reported that its business associate (BA) experienced a cyber-attack that affected the electronic protected health information (ePHI) of 22,579 individuals. The ePHI involved included names, dates of birth, health insurance and financial information, and treatment information. The CE notified HHS, affected individuals, and the media.
