OCR description
The covered entity (CE), On Lok Senior Health Services, reported that it experienced a ransomware attack that affected the electronic protected health information (ePHI) of 1,643 individuals. The ePHI involved included names, dates of birth, addresses, diagnoses/conditions, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. The CE also created a call center and offered complimentary credit monitoring services. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. OCR provided technical assistance to the CE regarding the HIPAA Security Rule.
