OCR description
The covered entity (CE), Nystrom & Associates, reported that several employees were the victims of an email phishing attack that affected the protected health information (PHI) of 985 individuals. The PHI involved included names, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical and administrative safeguards to better protect PHI.
