OCR description
The covered entity (CE), Adena Fayette Medical Center, reported that several employees were victims of an email phishing attack affecting the protected health information (PHI) of 1,389 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical and administrative safeguards to better protect its PHI and provided complimentary credit monitoring services to affected individuals.
