OCR description
The covered entity (CE), Ascension via Christi ACE, reported that employees of its business associate (BA) experienced an email phishing attack that affected the electronic protected health information (ePHI) of 1,977 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, diagnoses/conditions, and financial and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the BA implemented additional technical safeguards to protected ePHI.
