Electrical Workers Local 369

The covered entity (CE), Electrical Workers Local 369, reported that an employee was the victim of an email phishing attack that compromised the protected health information (PHI) of 662 individuals. The PHI involved included names, dates of birth, Social Security numbers, financial information, diagnoses, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE revised its policies and procedures and provided additional training to employees regarding email security.
