OCR description
The covered entity (CE), UnitedHealth Group Single Affiliated Covered Entity, reported that a coding error caused the protected health information (PHI) of 774 individuals to appear on its patient portal. The PHI involved included names and Social Security numbers. The CE notified HHS and the affected individuals. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
