OCR description
The covered entity (CE), Valley Physical Medicine & Rehabilitation, reported that an employee sent an email containing the protected health information (PHI) of 1,866 individuals to an authorized recipient. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE sanctioned the responsible employee, strengthened its administrative safeguards, and retrained its staff.
