Wisconsin Institute of Urology, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 4,819 individuals. The PHI involved included names, addresses, dates of birth, Social Security Numbers, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
