OCR description
Waste Management Health and Welfare Benefits Plan, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 922 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, and medications. The CE notified HHS, affected individuals, and the media. The CE strengthened its administrative and technical safeguards to better protect its sensitive data.
