OCR description
The covered entity (CE), Frisco Fertility Center dba Dallas IVF, reported that a workforce member impermissibly downloaded the protected health information (PHI) of 552 individuals onto a USB device. The PHI involved included names, addresses, dates of birth, phone numbers, health insurance and claims information, medical records, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE re-educated its workforce members and strengthened its administrative safeguards to better protect its PHI.
