Fairfax Medical Facilities, Inc.

The covered entity (CE), Fairfax Medical Facilities, reported that multiple employees were the victims of an email phishing attack that compromised the protected health information (PHI) of approximately 4,970 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, financial and health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained its employees on email security and implemented additional administrative, technical, and security safeguards to better protect its PHI.
