OCR description
The covered entity (CE), Alluma, reported that an employee mailed the protected health information (PHI) of 4,642 individuals to the wrong recipients. The PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative safeguards to better protect sensitive data.
