South Florida Community Care Network LLC d/b/a Community Care Plan

The covered entity (CE), South Florida Community Care Network, LLC, dba Community Care Plan, reported that an employee emailed the electronic protected health information (ePHI) of 48,334 individuals to her personal email account. The ePHI involved included names, addresses, dates of birth, gender, phone numbers, diagnoses, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its ePHI. All employees were retrained on the importance of protecting the privacy and security of sensitive information.
