OCR description
The covered entity (CE), Family Medical Center of Michigan, reported that it experienced a hacking attack that compromised the protected health information (PHI) of 21,988 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, treatment information, and diagnoses/conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented addition technical safeguards. OCR provided technical assistance.
