OCR description
The covered entity (CE), Beaumont Health, reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of approximately 1,568 individuals. The ePHI involved included names, addresses, birthdates, diagnoses, and other treatment information. The CE notified HHS while the BA notified affected individuals, the media, and provided substitute notice.
