Queen Creek Medical Center d/b/a Desert Wells Family Medicine

The covered entity (CE), Queen Creek Medical Center dba Desert Wells Family Medicine, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 35,000 individuals. The PHI involved included names, dates of birth, addresses, Social Security and drivers’ license numbers, claims information, diagnoses, lab results, medications prescribed, and other treatment data. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring and identity theft services and strengthened its administrative, technical, and security safeguards. OCR provided technical assistance to the CE regarding the HIPAA Rules.
