OCR description
The covered entity (CE), Lakeshore Dental, reported that an employee impermissibly accessed the protected health information (PHI) of 2,000 individuals. The PHI involved included, names, addresses, Social Security numbers, and birthdates. In its mitigation efforts, the CE strengthened its administrative safeguards to further secure PHI. OCR provided technical assistance regarding the HIPAA Rules.
