OCR description
The covered entity (CE), California Department of State Hospitals – Coalinga, reported that a workforce member impermissibly disclosed the protected health information (PHI) of 1,738 individuals. The PHI involved included names, dates of birth, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE amended its policies and procedures regarding handling patient records. OCR provided technical assistance to the CE.
