OCR description
The covered entity (CE), ADEC Incorporated reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 1,999 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims and financial information, diagnoses/conditions, lab results, medications, and treatment and health insurance policy information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. OCR provided technical assistance.
