The covered entity (CE), Eastern Connecticut Pathology Consultants, reported that multiple employees of its business associate (BA) were the victims of an email phishing attack that affected the protected health information (PHI) of approximately 5,978 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, financial and health insurance information, and prescription and other clinical information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
