OCR description
The covered entity (CE), Pathology Consultants of New London, reported that several employees of its business associate were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of approximately 835 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
