The business associate (BA), HBP Financial Services Group, reported that several employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of approximately 2,238 individuals. The PHI involved included names, address, dates of birth, Social Security numbers, health insurance information, and financial information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards to better protect PHI. Staff were retrained on email security precautions.
