OCR description
The covered entity (CE), Samaritan Center of Puget Sound, reported that a desktop computer and server that contained the protected health information (PHI) of 20,866 individuals, were stolen from its facility during a burglary. The PHI involved included names, addresses, Social Security numbers, health insurance information, financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following the breach, the CE strengthened its physical safeguards at its facility and implemented additional technical safeguards.
