OCR description
The covered entity (CE), Orlick & Kasper, MDs, reported that several employees impermissibly accessed and acquired the protected health information (PHI) of 30,000 individuals without authorization. The PHI involved included names, addresses, Social Security numbers, and treatment and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
