Horizon House, Inc.

The covered entity (CE), Horizon House, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of approximately 27,823 individuals. The PHI involved included names, dates of birth, drivers’ license numbers, addresses, Social Security numbers, claims and financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR provided technical assistance regarding the HIPAA Security Rule.
