OCR description
The covered entity (CE), GYN Emergent Care Center, reported that an employee lost a prescription logbook that contained the protected health information (PHI) of 5,080 individuals. The PHI involved included names, addresses, dates of birth, medications administered, and other treatment information. The CE notified HHS, affected individuals, the media, and established a complimentary phone number for questions or concerns.
