OCR description
The covered entity (CE), Cerebral Medical Group, reported that a workforce member sent an email containing the protected health information (PHI) of 875 individuals to the wrong recipient. The PHI involved included names, addresses, dates of birth, diagnoses, medications, and health insurance and other treatment information. The CE notified HHS and affected individuals. In response to the breach the CE retrained the workforce member on email security. OCR provided technical assistance to the CE.
