OCR description
The covered entity (CE), Aetna ACE, reported that an employee of its business associate (BA) was the victim of an email phishing scheme that affected the protected health information (PHI) of 1,011 CE patients. The PHI involved included names, dates of birth, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media.
