OCR description
The covered entity (CE), Epilepsy Foundation of Texas, reported that an employee was the victim of an email phishing attack that affected the protected health information (PHI) of 2,824 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, and clinical information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on email security.
