OCR description
The covered entity (CE), Zenith American Solutions, reported that an employee mailed postcards containing the Social Security numbers of 1, 907 individuals. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In response to the breach incident, the CE strengthened its administrative safeguards and retrained the responsible employee.
