OCR description
The covered entity (CE), Humana, reported that an employee of its business associate (BA) inadvertently mailed the protected health information (PHI) of 948 individuals to the wrong recipients. The PHI involved included names, addresses, dates of birth, diagnoses, conditions, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the BA strengthened its administrative safeguards to better protect its PHI.
