OCR description
The covered entity (CE), Drs. Kelley & McDowell, reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 6,204 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The BA notified HHS, affected individuals, the media, and posted substitute notice on its website. The BA also provided complimentary identity protection services. The BA implemented additional administrative and technical safeguards to better protect ePHI.
