OCR description
The covered entity (CE), ReproSource Fertility Diagnostics, was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 253,774 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, financial and claims information, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified OCR, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards.
