OCR description
The covered entity (CE), Orange County Health Authority, reported that an employee sent the protected health information (PHI) of 4,732 individuals to the wrong recipients. The PHI involved included names, addresses, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI. Employees were also retrained. OCR provided technical assistance to the CE.
