OCR description
The covered entity (CE), Clinical Pathology Diagnostics, reported that employees of its business associate (BA) experienced an email phishing attack that affected the protected health information (PHI) of approximately 2,500 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial and health insurance information, and prescription and other clinical information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
