UMass Memorial Health Care, Inc.

UMass Memorial Health Care, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 209,048 individuals. The PHI involved included names, dates of birth, and Social Security numbers. UMass notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, UMass implemented additional administrative, technical, and security safeguards. In addition, UMass retrained its workforce members on email security precautions.
