OCR description
The covered entity (CE), Walmart, reported that a laptop containing the protected health information (PHI) of 828 individuals was lost. The PHI involved included names, dates of birth, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its physical security measures and retrained its workforce members on policies and procedures to safeguard PHI.
