OCR description
The covered entity (CE), Seneca Family of Agencies, experienced a cyber-attack affecting the protected health information (PHI) of 3,900 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, phone numbers, email addresses, medical record numbers, diagnoses, health insurance information, prescription information, drivers’ license/state identification numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards.
