OCR description
California Physicians’ Services dba Blue Shield of California, a covered entity (CE), reported that its business associate (BA) experienced a ransomware attack that compromised the electronic protected health information (ePHI) of 2,841 individuals. The ePHI involved included names, addresses, dates of birth, and health insurance information. The BA notified law enforcement; the CE notified HHS, affected individuals, the media, and provided substitute notice.
