OCR description
The covered entity (CE), Nationwide Laboratory Services, reported that it was the victim of a ransomware attack that affected the protected health information (PHI) of 33,437 individuals. The PHI involved included names, dates of birth, medical record numbers, Social Security numbers, and clinical and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification and Security Rules.
