OCR description
The covered entity (CE), University of North Carolina Health reported that its business associate (BA) discovered that employees impermissibly accessed the protected health information (PHI) of 946 individuals. The PHI involved included the names, addresses, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE changed its computer system security settings to better protect its PHI and implemented additional technical safeguards and provided training to its employees.
